Success Story: Mrs. H.

Mrs. H admitted to Mission Palms of Mesa Health and Rehab on 5/19/19 following a bladder surgery due to a malignant neoplasm of the bladder. Prior to hospitalization and illness, Mrs. H was independent with all mobility and activities of daily living. Overall cognition was normal. Due to decline in overall functional levels and cognition at the hospital, Mrs. H was sent to Mission Palms of Mesa Health and Rehab to improve her overall strength, endurance, cognition and balance in order to improve her ability to complete self-care and mobility tasks more independently for safe return home with spouse. Upon her therapy evaluations for occupational, physical and speech therapy, she required minimal assistance for all activities of daily living, mobility and transfers. She also had difficulty with problem solving and short term memory with increased confusion per spouse. Following a 4 week course of skilled occupational, physical and speech therapy along with excellent nursing care, Mrs. H was able to return home with her spouse at a modified Independent to independent level using a FWW. Mrs. Herriman was set up to continue with home health therapy to continue to progress her abilities to return to complete independence in home setting.
